*First booth space is $200, additional spaces $150 each. Booths come with draped table, two chairs and garbage can. Add-ons are requested through the Dixie Center. Exhibitor packets are mailed one month prior to conference.

Spaces are temporarily reserved when this application is received. Reservation becomes official upon receiving payment. Payment is due by January 2, 2010. Make check payable to UMEA (sorry, but UMEA is not able to accept credit card payments at this time). Please send payment to:
